PRE-REGISTRATION CHECKLIST

2007 OMSA Invitational

The required items listed below need to be received no later than October 5, 2007 to complete your pre-registration:

Team roster to be registered on-line at our website location of www.oldmillinvitational.com

Waivers signed by each player’s parent or guardian

Copy of birth certificate or other proof of age document for each player (no original documents)

Please staple the waiver and birth certificate together for each player, as this will greatly help our verification process.

Check for $250.00 payable to Old Mill Soccer Association.

The team roster will need to be registered on-line. The waivers, birth certificates and check are to be mailed or delivered to:
